Disclose diagnosis at new job?

DS just got a new job at a public high school and started a few weeks ago. They gave him a form to fill out and one of the questions asks directly if he has any chronic illness. On one hand I think he should put Not Applicable because he’s stable (thank goodness) and not asking for any accommodations. On the other hand I don’t want something to happen down the road and have them think he lied.

He also has to go on their health insurance so THAT is giving me anxiety hoping that he’ll still…
