Adolescents with chronic conditions face complex challenges during transition to adult care that often result in adverse health outcomes. Global best practice standards exist, but their implementation varies. Urgent, collaborative action, such as establishing youth-centred structured transition programmes, is needed to ensure adolescents are empowered to manage their health and thrive.
